We are a State Licensed, Medicare/Medicaid Certified and Joint Commission Accredited Hospice Care provider. The mission of Access TLC Hospice is to provide quality hospice care to patients with a limited life expectancy and emotional support to their families. The primary goal is to improve the “Quality of Life when the Quantity of Life is limited”sm. The Hospice program encompasses the physical, emotional, spiritual and social needs of the patient.
Our “goal” at Access TLC Hospice is to help our patients continue life with minimal disruption of normal activities, live life fully, with privacy and dignity in their own home or in a skilled facility and provide bereavement support to the family after the patient’s death. Our goal is to give patients and their families “Quality of Life” by allowing them to control their destiny.
Typically, a family member serves as the primary caregiver and, when appropriate, helps make decisions for the terminally ill individual. Members of the hospice staff make regular visits to assess the patient and provide additional care or other services. Hospice staff is on-call 24 hours a day, seven days a week.
