Sangre de Cristo Community Care is a non-profit organization providing compassionate care to patients and families facing rehabilitation, chronic or serious illness, and end-of-life challenges. With services including hospice, home health, dementia care, palliative care, and bereavement support, Sangre is committed to preserving quality of life and dignity. We serve all terminally ill patients and their families—regardless of background or place of residence. Our hospice team offers care at home, in hospitals, extended care facilities, or at our in-patient facility, the Joni Fair Hospice House. We proudly offer services in Pueblo, Cañon City, La Junta, Trinidad, and Walsenburg.

“I have been a caregiver for the same client for over 5 years. Sangre de Cristo has been her visiting nurse care since before I started taking care of her. Our current visiting nurse, Kayla Wheeler, is by far our favorite nurse. We prefer to only have her to take care of whatever is needed. Kayla is professional always. She shows genuine concern whenever there is an issue or problem. She is friendly, attentive, and kind. When we have questions, she does her best to answer. And if she doesn't know the answer, then she finds someone who does. She makes sure to inform us of when our next visit is going to be, and if we have conflicting schedules she makes sure to work out something to be sure that nothing falls through the cracks. I cannot say enough how impressed we have always been with Kayla, I really hope that she continues to care for my client for as long as she needs her ❤️”
