Rocky Mountain Healthcare Community transitional care is traditionally for patients still needing specialize care after being discharged from the hospital. Our program restores hope, can accelerate recovery, helps avoid re-hospitalization and enables many to return home. Rocky Mountain Healthcare Community provides 24-hour care by registered nurses, licensed nursed, and certified nursing assistants. For many, a primary component of their stay is therapy. Our physical therapist, occupational therapists, and speech-language pathologists use state-of-the-art clinical strategies and equipment to help patients reach their goals.
