We provide the following servises:
• New and existing tube feedings, (Gastrostomy, PEG) with nutritional supplements, and supplies.
• IV fluids for hydration (Supplies included).
• Bowel and bladder training (Supplies included).
• Amputee care & training.
• Diabetic Management and supplies.
• Assessment and management of chronic disease and co-morbidities that do not complicate the
treatment plan.
• Pain management, SQ, IM (Standard High-Cost Drugs Excluded).
• Nebulizer treatments, 1-2 treatments / day. (Low level respiratory care needs depending on the
other complexities and needs of the patient, can be reviewed on a case-by-case bases with
Nurse Case Manager to determine flexibility in the agreed and most appropriate authorization
level and re-authorization levels, Supplies Included).
• Administration of antibiotics (No IV included in this level).
• Comprehensive care reflects moderate nursing intervention.
• Active treatment of co-morbidities with potential for co-morbidities to affect treatment plan.
• Diabetic management.
• Dialysis patients (Dialysis, Transport and Off-site Licensed Nursing / Respiratory Staff, not
included).
• In and out cath care and supplies.
• New and existing ostomy care, training, and supplies.
• Wound care stage I and II, superficial, or post-surgical dressing changes not to exceed 1
treatment / day (Supplies included) (Multiple Wounds are included under level 2).
• Stage I and II decubs not to exceed 1 treatment / day (Supplies included) (Multiple Wounds are
included under level 2).
