A Congregate Living Health Facility is a residential home which provides inpatient care, including the following basic services: medical supervision, 24-hour skilled nursing and supportive care, pharmacy, dietary, and social recreational. Proactive Care Inc. provides skilled nursing care on a recurring, intermittent, extended, or continuous basis. The care is generally less intense than that provided in general acute care hospitals, but more intense than that provided in skilled nursing facilities.
MIH Project A provides services for persons who are catastrophically and severely disabled acquired through trauma or non-degenerative neurologic illness, for whom it has been determined that active rehabilitation would be beneficial and for whom these services would be provided. Our Care Team will provide fulfilling, healthier living, and homelike environment to make our residents feel at home.

MIH Project A, LLC Registered Nurses (RN), Licensed Vocational Nurses (LVN), Certified Nursing Assistants (CNA), and contracted providers offer the following services to our residents based on medical complexity and therapy needs. Our Care Team offers the following services to our residents:
Brain injuries such as, traumatic, anoxic, aneurysms/bleeding, cerebral vascular accidents.
Amputee Care
Colostomy Care
Catheter Care
Wound Care
Rehabilitation Care
Trauma Care
Pulmonary Care including ventilator and tracheostomy care
Medically Complex Cases such as trauma or fracture, residents with multiple antibiotic therapies, and tube feedings.
