Fountain Hospice Services are available in the patient’s home, nursing homes, assisted living facilities, and inpatient units.

24/7 Hospice Medical Director On Call who works as a member of the hospice team
24/7 Specialized Skilled Nurses trained with symptom management
Certified Home Health Aids readily available to assist with activities of daily living
Social Workers who can provide community resources and counseling
PT/OT/ST as needed
Nutrition Management Counseling
Psychological and Bereavement Counseling
Inpatient Skilled Nursing Facility Care as needed for pain or symptom management, respite, continuous care for crisis situations
Fountain Hospice specializes in diagnosis management of Cancer, Dementia, COPD, CVA, Liver/Renal Disease, Pulmonary Disease
