College Oak Nursing and Rehabilitation Center is a 120-bed Skilled Nursing Facility located in the North Area of Sacramento specializing in the care and rehabilitation of seniors recovering from a hospital stay.
Our goal is to help residents achieve their highest practical level of functioning by combining his or her lifestyle preferences with sound medical treatment. To this end, personalized plans of care are developed by our staff in conjunction with the resident and/or family and with the guidance of the resident's physician.
