MEND: (Mental Health Education & Skilled Nursing Disease management): our Mental Health program provides a wholistic approach to pt’s care. We have a multidisciplinary team that is comprised of PT, OT, SLP, MSW, Dieticians, Journey Care, our skilled nurses, Mental Health Registered Nurse Consultant and Psychiatric Medical Director.
