Inpatient Falls

I am looking for advice on reducing inpatient falls… It seems the more we try, the more falls occur. Most of the falls are occurring on the medical-surgical unit. Although our fall rates for the year have been high in our ICU and SNF units as well. We have currently formed a sub-committee to review each fall and work towards ways to prevent in the future. Any thoughts are greatly appreciated!

SNF Placement with Extra Costs

Looking for some thoughts or experiences…… A patient ready for SNF placement and has a very costly antibiotic plus costs of daily transport for radiation treatments. Patient’s preferred SNF refused based on costs willing to consider if patient pays the extra antibiotic costs. Another SNF has agreed to cover antibiotic costs but patient may need to cover cost of daily transportation. Have you ever had an SNF conditionally agree to accept patient if he/she shared the costs? Are there any issues w/medicare & the patient being required to share costs? We generally think of the first 20 days being a totally covered period. Thanks.

Is anyone aware if there is any regulation / requirement to offer a patient and their family choices when recommending acute rehab? Does freedom of choice apply in this instance, like it does for SNF?


Skilled Nursing Facility – Important Message Medicare

Can anyone tell me if a Skilled Nursing Facility patient requires an Important Message from Medicare? Per the CMS site and Learning Link it only says Inpatient.

SNF Placement

Greetings everyone. I need your advice. I am dealing with a family that has ignored/ rejected every single SNF suggestion I have ever made. I think they are a little bit unrealistic with what they want and what is actually possible. We are in disagreement here and I am not sure what to do. I am thinking of arranging a deal with them. Can I tell them that SNF A has a bed and the family member will be transferred there BUT they can make their own arrangements to transfer them to another SNF of their choice (if and when they ever find one that suits their standards). Is this acceptable or am i being too sarcastic? Thanks for all you help and very informative info

Has anyone developed a tool that detects high-risk readmission from a SNF or a Hospital?

In your opinion are there enough case management departments taking ownership of this new initiative?
