Transitional Case Manager Role

We are expanding our case management department to incorporate transitional case managers – to collaborate with physician offices, SNF’s, Assisted Living facilities in our community, with the goal of preventing re-admissions within 30 days of discharge. We are also looking at ways to decrease use of our E.D. as a source of “primary care” for those in the community without primary care physicians.
Please let me know if you have any experience with this aspect of case management, including your policy and procedures, etc.

Hospice Questions

Interested in some feedback….

1. For those who provide inpatient hospice care, but do not have a dedicated hospice unit within the hospital, do you discharge the patient from inpatient to hospice care?
2. Do you consider this a hospital readmission if a separate hospice provider number is not available?

To Combine or Not to Combine..

If a Medicare patient left AMA, came back and then was readmitted (for the same reason) the same day as the discharge, normally this would need to be combined. However, does the fact that they left AMA change the rule? Thank you in advance for your time.


Readmission Reviews

How are others reviewing Readmissions – Medicare and other payers? How much of the decision to combine or bill separately is based upon clinical judgement of reviewer and how much is based upon written standards / guidelines?

If anyone has documented standards that you are willing to share it would be greatly appreciated. At what point are you consulting with EHR or other Physician Advisor?


Combined Admissions

I was at a recent meeting where combined/separate admission determinations were discussed. I am wondering what the thoughts out there were on this. Most of our readmissions are billed as separate bills in following the Medicare readmission guidelines but we still have some patients who fall into the combined billing category. Any thoughts?


Another readmission prevention question

In your facilities do you have a certain protocol or strategy for what is included in the reducing readmission plan? Now there are more comprehensive ones including NP transition coaches, making F/U appointment with care provider within 7 days, and including pharmacists in providing d/c instructions. Are there any other disciplines that should be included such as MSW, OT/PT, others?

Has anyone developed a tool that detects high-risk readmission from a SNF or a Hospital?

In your opinion are there enough case management departments taking ownership of this new initiative?
