Clinical Ladder / Role Advancement

I am interested to know who uses a clinical ladder or a type of role advancement within their department?

An example might be: an entry level case manager might be a case manager 1, but in time can progress to a case manager 2, who holds a BSN and/or case management certifications, sits on committees, meets some other professional benchmark and then can advance to a case manager 3 for additional professional benchmarks.

If you utilize a staff development system that is in any way similar please share. If there is any supervision duties within your matrix I am interested in that also.

Chronic health problems

So many people have multiple long term issues with Diabetes, obesity, hypertension and hypercholesterolemia. How do you help these people get into a lifestyle program to improve their health or prevent more complications from the above? The Adult Day Care population I’ve worked with is mostly Hispanic and most of the clients have at least 2 of the above if not all of the above diagnoses. I’ve tried counseling and exercise programs and just haven’t made any significant progress. These issues have become major health problems for America. How can we begin the fix this?

Dear Colleagues in California,

I was wondering if anyone can help me clarify who is responsible for getting the PASRR completed. Is this the duty of the SNF or Case Management?


RN Case managers doing UR?

Currently, we have our RN Case Managers doing utilization reviews along with their other duties (discharge planning, etc). I know that some systems have chosen to have these roles done by separate people. I would be interested to hear the advantages and disadvantages of having them separate. If anyone is willing to share their perspective I’d really appreciate it/ Carrie
