Discharge Planning

asked April 3, 2014 | 0 followers | follow | 4 answers

I am seeking information on discharge planning processes. At your facilities, who obtains precerts for discharge meds and/or arranges for home IV infusions? Are these handled by the RNs, SWs or “techs”? Thanks for the information.

Is there any sort of protocol or procedure for conducting these when a patient is about to be discharged? Are these becoming more used in the hospital and not just on the Acute Rehab floor?

Early Discharge Time

asked July 8, 2013 | 0 followers | follow | 2 answers

Good morning-

Does anyone have any significant experience in obtaining earlier discharge times for patients? We currently have an issue with our discharges occurring from 4 PM to 7 PM each date and I am wondering if anyone has had any experience in addressing this problem.

Any advice/thoughts/ideas would be appreciated.

JCAHO Survey

asked June 10, 2013 | 0 followers | follow | 3 answers

A recent survey had a question that said “How is the discharge plan brought into the plan of care?” Can someone explain how to accomplish that? I think I have an idea but would like some feedback. Thanks!

Another colleague proposed this question: Is there a template of some sort being used for assessment and follow-up of CHF/chronically ill patients during a discharge?

Post Discharge

asked May 31, 2013 | 0 followers | follow | 0 answers

I have an admitted patient who needs a very expensive test performed but the results won’t be back before discharge. I want to know if he can go to the lab immediately after discharge to get the test done with the hospital getting reimbursed separately from their DRG payment. Is there any “3-day rule” which corresponds to the 3-day rule prior to admission?

Day Surgeries

asked May 28, 2013 | 0 followers | follow | 1 answers

I have a patient scheduled for a “day surgery” (but very late in the day) procedure with appropriate recovery hours and I need to clarify something. Do you discharge him when stable for discharge regardless of the time of night(very early am hours)?? Our day surgery unit closes at 9:30 pm and patients are transferred to a nursing unit for care.

Placement for Sex Offenders

asked May 23, 2013 | 0 followers | follow | 3 answers

Hi guys, I have a patient who is a registered sex offender and I can’t find a facility that will take him in. Does anyone know of any nursing facilities (post acute care LOC) who accept people that are registered sex offenders? I am in Lincoln, NE but I am open to all options…doesn’t really matter what state at this point. It would be great if anyone can point me to the right direction.

Does anyone here have any experience working with this program? http://www.walgreens.com/healthcare/business/ProductOffering.jsp?id=wellTransitions
Is their discharge follow-up separate from or coordinated with the hospital?
