Transitional Case Manager Role

We are expanding our case management department to incorporate transitional case managers – to collaborate with physician offices, SNF’s, Assisted Living facilities in our community, with the goal of preventing re-admissions within 30 days of discharge. We are also looking at ways to decrease use of our E.D. as a source of “primary care” for those in the community without primary care physicians.
Please let me know if you have any experience with this aspect of case management, including your policy and procedures, etc.

Changing IP to OBs

If you have a patient who was admitted IP and was justifiably IP but then ceased to meet IP criteria, lets say after day 3. Do you change the status to observation/outpatient? I have always thought, no you wouldn’t change the status, but if you don’t change the status do your CM’s do a concurrent review and not cite any criteria? Please let me

Physician Based Case Manager

We are planning to implement physician based case managers for our hospitalists. In trying to map out the logistics, I am hoping to learn from my colleagues on how this can be done efficiently and effectively. We will be utilizing a hybrid case management model, since the only physician based case managers will be exclusively for the hospitalists and the balance of the case managers will remain unit based. Any insight, processes, and lessons learned about physician based CM would be greatly appreciated. Thank you in advance.


Review for Accuracy

How does everyone review case managers for accuracy on Interqual criteria. For example, peer chart reviews? Do you blindly give them charts that have been looked at by this person and see if another comes up with the same answer? I am really trying to develop some objective evaluation criteria for my RN Case Managers. I would appreciate any suggestions that anyone has.

Clinical Ladder / Role Advancement

I am interested to know who uses a clinical ladder or a type of role advancement within their department?

An example might be: an entry level case manager might be a case manager 1, but in time can progress to a case manager 2, who holds a BSN and/or case management certifications, sits on committees, meets some other professional benchmark and then can advance to a case manager 3 for additional professional benchmarks.

If you utilize a staff development system that is in any way similar please share. If there is any supervision duties within your matrix I am interested in that also.
